How do I schedule a facility tour?
Scheduling a tour is simple. You can use the 'Inquire Today' or 'Refer a Client' buttons on our site, or call us directly at 612-404-5110. We will work with you to find a convenient time window during the week.
What is the admissions process?
Admissions begin with a clinical review and a coordination meeting. We partner with the resident, their family, and the county waiver team to ensure a successful transition from a hospital, TCU, or home setting.
Do you accept CADI, BI, and EW waivers?
Yes. Eden Health Care specializes in supporting individuals funded through CADI, BI, and EW waivers. We also welcome residents through Private Pay agreements and coordinate closely with waiver case managers.
What level of staff supervision is provided?
We provide 24/7 awake staff supervision. Our clinical team is dedicated to continuous health observation, behavioral redirection, and full assistance with activities of daily living (ADLs) in our 4-bed home.
